Résumé : Le traitement HIT des voix des patients souffrant de schizophrénie
peut réduire durablement les hallucinations auditives persistantes et leur
fardeau : résultat à 18 mois d’une étude randomisée contrôlée
Objectif : Cette étude visait à analyser le résultat d’une étude randomisée
contrôlée (ERC) de 18 mois sur le fardeau subjectif et la psychopathologie
des patients souffrant de schizophrénie.
Méthode : Une ERC a servi à comparer le traitement intégratif axé sur les
hallucinations (HIT) et le traitement de routine (TR) chez les patients
souffrant de schizophrénie qui entendent des voix de façon persistante.
Une analyse des sujets retenus au début de l’essai a été effectuée sur
chacun des 63 patients, qui ont été évalués au départ, à 9 mois et à 18
mois. À chacune de ces 3 occasions, les effets différentiels des conditions
du traitement ont été testés à répétition. Le sexe, l’âge, le niveau d’instruction
et la durée de la maladie (hallucinations) ont été utilisés comme covariables.
Résultats : Les patients du groupe expérimental ont conservé les améliorations
avec le temps. Les améliorations des hallucinations, de l’angoisse et du
contenu négatif des voix sont demeurées significatives au niveau de 5 %.
Conclusions : Le traitement HIT semble être une stratégie efficace avec des
effets durables pour les patients réfractaires au traitement qui entendent
des voix.
